Ecu - allow me to explain something.
We are now governed by a coalition. When a coalition is formed neither side of that coalition gets to keep everything in their manifesto, but they have to present a unified front on all legislation which is supported by the cabinet...
If the referendum is passed in favour of AV you will find it is sponsored through parliament by the govt, and all govt ministers will be supporting it.

Furthermore, ecu, the coalition's NHS legislation doesn't exist as yet.  It is still a Bill making its way through the Commons, having had its 1st and 2nd readings during January, and gone through its Committee debate stage during March.  It is now entering its Report stage.  It will still have to have its 3rd reading, work its way through the Lords, and deal with any issues that that House raise about it.
